Patients and Legal firms may request medical reports by submitting a Medical Records Request online.
In Person (patients only): Visit Fire Administration, 1525 Arkansas Lane, Monday - Friday, 7 a.m. - 4 p.m. (Patients will be required to complete a Medical Request Form and will need to provide identification.)
